
The real Roman carbonara — no cream, no shortcuts. Spaghetti tossed in a silky sauce made only from egg yolks, whole eggs, Pecorino Romano, guanciale, and black pepper. A classic comfort dish with accurate macros so you can bolus confidently.
Bring a large pot of generously salted water to a rolling boil. Cook the spaghetti according to the package directions until al dente. Reserve 250 ml (1 cup) of starchy pasta water before draining.
While the pasta cooks, cut the guanciale into strips about 1 cm (½ inch) wide. Place them in a cold, large skillet and cook over medium heat, stirring occasionally, for 6–8 minutes until the fat has rendered and the guanciale is golden and crispy. Remove the pan from the heat.
In a bowl, whisk together the 4 whole eggs, 2 extra yolks, and the grated Pecorino Romano until smooth and creamy. Add a generous amount of freshly ground black pepper and whisk again.
Add the drained spaghetti directly into the skillet with the guanciale (off the heat). Toss vigorously so the pasta is coated in the rendered fat.
Pour the egg-and-cheese mixture over the pasta and toss very quickly and constantly for about 30 seconds. The residual heat will cook the eggs into a silky cream — do NOT return the pan to direct heat or the eggs will scramble.
If the sauce is too thick, add a splash of the reserved pasta water (a tablespoon at a time) and toss until you reach a glossy, coating consistency. Serve immediately with extra Pecorino and black pepper on top.
Carbs come from white pasta (GI ~70) — pre-bolus 15–20 minutes before eating and use your usual dosing method agreed with your care team for precise dosing. The high fat content from guanciale and egg yolks significantly delays glucose absorption — consider a dual-wave or extended bolus over 2–3 hours, and check your blood sugar at both 2h and 4h post-meal.
